The Special Meal Request Form is to be completed by the parent/guardian and signed by the student's licensed medical provider.
Please distribute this paperwork to families requiring meal modifications for the upcoming school year. All completed forms can be sent to nutrition@maschiofood.com or faxed to (908) 888-2335.
